This file stores all the ICD procedure and diagnostic codes and
the CPT codes used to identify patients for a given registry during the
Registry Update process. The B cross references for the ICD CODE, ICD
PROCEDURE CODE and the INPATIENT CPT CODE are used in the EXPRESSION of the
ROR SELECTION RULES file (#798.2). The file design allows CCR to support
an unlimited number of codes selected from the ICD DIAGNOSIS file (#80),
the ICD OPERATION/PROCEDURE file (#80.1) and the CPT file (#81).
