The unauthorized claim has been received at the facility, but is pending
review to determine if the claim is complete, in which case it would
progress to COMPLETE/PENDING REVIEW status, or if further information is
required, in which case it would progress to INCOMPLETE status.
A claim in this status, INITIAL ENTRY, is not currently being acted upon.
A parameter in the FEE BASIS SITE PARAMTERES file determines if this status
is used. The activation of the status will override the parameter.
