Assess causative factors, including nutrition.
Monitor for signs and symptoms of hyperglycemia/hypoglycemia and blood glucose level
If blood sugar is less than 40 or greater than 500, implement protocol.
Provide education to patient/family regarding disease process, nutrition, medications and exercise.

DESIRED OUTCOME
This is a collaborative problem that describes a person who is
experiencing a blood glucose level that is too low or too high for
metabolic function. This does not apply to chronic diabetes that is
adequately controlled.
