- Assess for causative and contributing factors.

- Instruct patient to take several deep breaths, avoid sudden changes in position to prevent vomiting.

- Provide comfort measures: Reduce or eliminate noxious stimuli, apply cool, damp cloth to patient's forehead, provide oral care after each episode of emesis, and assess for the need of pharmacologic measures.

- Protect patient from risk of aspiration.

- Assess for fluid and electrolyte loss.

- Promote nutritional status: encourage intake of fluids and refer to dietitian.

- Provide education to patient/family regarding techniques to reduce nausea and vomiting, encourage intake of fluids, and potential for fluid loss.
