  • Collaborate with interdisciplinary team, as needed.
  • Educate patient/family regarding the type of fluid volume imbalance being experienced, and the appropriate interventions including dietary modifications, daily weight, and prescribed medications.
  • Identify contributing and causative factors including hemorrhage, GI bleeding, gastric suctioning, wound drainage, edema, hypervolemia, dehydration, increased intracranial pressure, compartmental syndrome.
  • Monitor vital signs, intake and output, lung sounds, daily weight, and electrolytes, and notify physician with abnormalities or changes.

DESIRED OUTCOME
A decrease, increase, or rapid shift from one to the other of 
intravascular, interstitial, or intracellular fluid; this refers to the 
loss, excess, or both, of body fluids.  This can represent several 
clinical conditions including hemorrhage, dehydration, edema, 
compartmental syndrome, increased intracranial pressure, or GI bleeding.
