\nA Super Team is defined as a team composed of two or more\ncomponent teams. It may be called a "Team of Teams." The\nability to "nest" teams within a super team allows the facility\nto associate a number of specialty or other teams for\nscheduling, availability and other purposes.\n\n
\nA non-Primary Care Team that is used specifically for OIF and OEF\nTransition assistance. The information in PCMM related to the OIF/\nOEF Team shall be transmitted to Austin in the same manner as\nPrimary Care Team information is transmitted. This information\nwill be monitored and tracked by the VSSC.\n\n
\nThe Case Manager is responsible for collaborating with services internal \nachieving desired outcomes for discharge, post hospital recovery and \nhealth maintenance/wellness.\nor external to the VA to manage the care of patients with needs of a \ncomplexity level that exceeds the capability of the Primary Care Teamlet \nto manage. The Case Manager is responsible for collaborating with \nservices external to the VA to facilitate care transition in order to \neffectively meet the patients' needs, provides patient and family health \neducation with a focus on self-management, prevention, and wellness based \non the patients goals, and collaborates with patients to assess and \nidentify needs, issues, care goals, and resources in critical factors for \n\n
\nPrimary Care teams staffed by non-VA providers.\n\n
\nPrimary Care teams providing Home Based Primary Care.\n\n
\nThe coordinated interdisciplinary provision of health\n(from IL 10-93-031, Under Secretary for Health's Letter)\ncare in regard to:\n(1) Intake and initial needs assessment, (2) Health \npromotion and disease prevention, (3) Management of\nacute and chronic biopsychosocial conditions, \n(4) Access to other components of health care, \n(5) Continuity, and (6) Patient and non-professional\ncare giver education & training.\n \n\n
\nA Team (usually multi-disciplinary) whose main purpose is\nto provide care for inpatients.\n\n
\nA Team consisting of Mental Health Clinicians and staff\nwhose purpose is to provide services associated with Mental\nHealth (Psychology/Psychiatry/Social Work, etc.)\n\n
\nA multi-disciplinary Team which is called in for immediate\nresponse to a "Code" (emergency assistance) or Psych crisis.\n\n
\nTeam members for Community Care work outside the VA facility\nand provide patient care via Community care centers set up\nby VA Medical Centers.\n\n
\nTreatment programs of a confidential nature. Extra care\nshould be exercised in regard to release of information\nand other facets of health care provision.\n\n
\nTreatment teams which are organized to provide Specialty\n(Medicine & Surgery) and Subspecialty (Cardiology, Dermatology,\nPodiatry, Peripheral Vascular, etc) care. These teams follow\ncertain patients, but also receive referrals from the Primary Care\nPractitioner.\n\n
