This will return the user's name if the user is an active dental
provider.  If the user is not, then it returns -1
  This will return the SIGNATURE BLOCK PRINTED NAME if it is present in
the NEW PERSON file.
  If not, then itr returns the .01 field value from the NEW PERSON file in
first m last format.
