| EDUCATIONAL STANDARDS |
1. The Care Coordinator will assign a Disease Management Protocol
(DMP) appropriate for the veteran upon enrollment.
2. The Care Coordinator will monitor the Veteran's responses to the
DMP questions to provide interventions when necessary.
3. Interventions are designed to enhance the Veteran's ability to
self manage his/her disease process.
