  - Last four digits of Patient SSN
  - Procedure Date/Time
  - Procedure Code
  - Procedure Name
  - Procedure Volume
  - Provider Name
 
One DSS Unit, multiple DSS Units, or all DSS Units can be selected.
specified location and DSS Unit(s) within a selected date range.
 
This report may be sorted by procedure date or patient name.  The output
includes:
 
  - Location 
  - Clinic
  - DSS Unit
