 - Matrix showing how many times a specific procedure was performed on a
   specific patient with the selected provider as Provider 1, Provider 2, 
   to Provider 7
 - Modifier(s)
 - Subtotals by procedure
 - Grand Total of procedures
 
This option allows you to print/display workload within a specified date
range for a selected provider.  The output includes:
 
 - Procedure code
 - Procedure name
 - Patient name
 - Last four digits of patient SSN
