Inclusion of additional fields for HL7 message transmissions to AAC.
Also removes reference to 'DSS' in mail group SD PM NOTIFICATION TIU.
The routine SDPMHLS, responsible for creating the structure of the HL7
message, has been modified to allow inclusion of data for
progress notes pending signatures and encounters without progress notes.
Totals for unique encounter providers and unique stop codes at the
division level should also now match the sum of the totals for these
values of each facility in that division.
