Patch makes the Clinic Field (#4.5) a required field in the Recall 
Reminders file (#403.5) and includes post-init routine SD53P566 which 
will provide a report of all entries in file #403.5 that do not have a 
clinic.  Also prohibits users from entering patients into the Patient 
file (#2) through the SDRR CARD ADD and SDRR PATIENT INQUIRY options.  
Finally, the patch includes a couple of fixes to the SDRR PATIENT INQUIRY 
option - displaying "User Who Entered" field incorrectly and displaying a 
Date Reminder Sent for all a patient's reminders, even those that have 
not been sent yet.
