FEE BASIS VENDOR file #161.2 lookup    ICR (5581)

The ECME package needs to look up entries and addresses in the FEE BASIS
VENDOR file (#161.2).  The lookup is by the "NPI" cross-reference.  The
desired fields are all on the zero node.

The fields:

2         STREET ADDRESS (F), [0;3] 2.5       STREET ADDRESS 2 (F), [0;14] 3
CITY (RF), [0;4] 4         STATE (RP5'), [0;5] 5         ZIP CODE (RFX),
[0;6]

This Integration Agreement is to be used to examine the "NPI" cross-reference
and to read the address fields.
