HBHCRP4 (7133)    OPTION (19)

This option prints the HBPC Visit Data by Date Range Report.  The
report is sorted by provider, then by visit date and includes:
Provider Name, Visit Date, Patient Last Name, SSN, DX, & CPT Codes with
Modifiers, with totals by provider & all providers total if All providers
included on report.  Report prints in 80 column format.
