The purpose of the Veterans Choice progress note is to communicate 
Post-Visit.
 
Pre-Visit Section: This section is used to document Veterans Choice 
Eligibility Type, Appointment Date and Time, non-VA provider, Approved 
Services, and any clinically pertinent information required for care 
coordination. 
 
Eligibility Type: One Eligibilty type must be selected per visit.

Veterans Choice appointment information and services approved. Its
 
Post-Visit Section: This section is used to communicate that documentation
from the non-VA provider has been received and scanned into the medical 
record. It is also used to document any post appointment outcome 
information that is clinically pertinent and required for care 
coordination. 
 
***The Post-Visit Section should be added as an addendum to the original
note that contains the Pre-Visit Information. Use the following 
instructions to open the VETERANS CHOICE NOTE template when making an 
primary purpose is for care coordination, but it is also to be used for 
addendum:
 
1. Select the original completed VETERANS CHOICE NOTE from the list of 
completed notes on the Notes tab in CPRS.\\ 
2. Right click in the note text and select "Make Addendum..."\\ 
3. When the blank Addendum opens, click on the "Action" menu above the
patient's name and click on "Reload Boilerplate Text".\\ 
4. The VETERANS CHOICE NOTE template will open.\\ 
5. Select "Post-Visit Information" and complete as appropriate.\\
attaching scanned documentation received from non-VA providers. 
 
Responsible Staff: The Veterans Choice Note will primarily be completed by
Non VA Care staff involved in care coordination with the Veteran.
 
Sections: The Veterans Choice Note has two sections, the Pre-Visit and 
TYPE dialog element
