The Annual Home Visit is to be conducted by a VA clinician.
for at home.
 
If the Veteran has a NEW PRIMARY CAREGIVER, the Veteran must complete a 
NEW APPLICATION to the CAREGIVER SUPPORT PROGRAM.  An Initial In-Home 
Assessment is required once the new Caregiver has been approved and 
completed the Caregiver training.
 
The assessing clinician is not responsible for developing a care plan or 
for the management of identified conditions unless an imminent safety 
risk is present.  The assessing clinician is responsible for 
communicating findings to the Nurse Care Manager or other appropriate 
clinician on the Veteran's Primary Care Team for necessary medical 
follow-up and to the CSC.  The primary intent is to be supportive of the 
Caregiver, and to maximize the likelihood that the Veteran will be cared
