This contact does not require a home visit and may be conducted by
Veteran's Treatment Team. It is expected that the staff completing the
assessment will review the Veteran's chart for hospitalizations,
emergency visits, outpatient visits, missed appointments, etc., prior to
the contact. It is also expected that staff will personally interview both
the Veteran and the Caregiver to complete the Interim 90 Day
Monitoring Assessment.
The outcome of the Interim 90 Day Monitoring Assessment is to be
communicated with the Veteran's CSC and Treatment Team, which will make
further determinations on whether or not an Interim In-Home Assessment
phone contact, Clinical Video Telehealth (CVT) or Face to Face contact
visit is to be completed.

with the Veteran's Treatment Team, the Veteran, and the Caregiver, with
consideration of the high risk criteria which may indicate the need for
a home visit.
