| DESCRIPTION | This Reminder Term is being used as an exclusion to homelessness
590, 591, and 592.
2. A Health Factor ("ALREADY RECEIVING ASSIST WITH HOUSING") used in the
dialog to indicate Veteran is receiving homelessness assistance from a
source not captured by a local visit in a clinic with a Stop Code
included in the Reminder Location List.
3. A Computed Finding ("VA-PROGRESS NOTE") which was actually entered
three times (X3). This was included to allow sites to enter one or more
than one progress note title that is being used by local staff dealing
screening. With a Beginning Date/Time set to 6M, if any finding that would
with Veteran homelessness, but NOT in a clinic associated with a
homelessness stop code. If Primary Care Staff, Social Workers or other
staff are providing those services in a different stop code clinic,
you may enter the note titles they use, and any of those titles will
exclude the Veteran from screening for 6 months.
To enter titles to exclude the Veteran from screening, simply edit one of
the CF VA-PROGRESS NOTE Findings, and enter the EXACT Title in the
"COMPUTED FINDING PARAMETER" field. If in the unlikely event that more
than three (3) PN titles are in use locally for this purpose, enter that
exclude the patient, is older than 6 Months, it is NOT found, and
CF again as another finding, by typing "CF.VA-PROGRESS NOTE" (IN QUOTES),
at the Finding: prompt.
therefore, will NOT have an effect on the reminder cohort - will no longer
exclude the patient from the cohort. The RT contains:
1. A Reminder Location List ("VA-HOMELESSNESS STOP CODES") that includes
all the Stop Codes indicative of a local visit in a VA Homelessness
clinic. Stop Codes included: 504, 507, 508, 511, 522, 528, 529, 530,
