      to prompt that discussion. 
    * Modification of taxonomy VA-BREAST TUMOR 
    * Addition of health factors that can be used by providers to indicate
      that screening is not clinically indicated because the patient is
    * Women between the ages of 50 and 74 have a mammogram every two
      unlikely to benefit from screening due to an estimated life
      expectancy of less than 5 years or sever comorbidities. 
    * Addition of branching logic to the dialog
 
US Preventive Services Task Force (USPSTF) also recommends that women age
50-74 have a mammogram every 2 years.
 
This reminder does not apply to women who have had a bilateral mastectomy.
 
Evidence of mammography performed will satisfy this reminder for 1 to 
      years
2 years depending on the screening frequency specified by the clinician. 
The clinician can alter the screening frequency for individual patients,
based on prior results, patient and family history, and discussions with
the patient.
 
Mammograms completed may be documented as one of the following and will 
resolve this reminder:
    Radiology result
    Women's Health procedure result
    Health Factor (Historical outside Mammogram result)
    * The decision to start regular screening every two years with 
    PCE CPT procedure code 
    Completed consult order for outside procedure
 
Screening will be resolved for 90 days or until results are recorded to
indicate the mammogram has been completed:
    Ordering a mammogram
    Health factor documenting a mammogram order
    Patient declined a mammogram 
    Clinician deferred a mammogram
 
      mammography for average risk women ages 40-49 should be an
Clinicians may manage follow-up screening to occur every 4 or 6 months or 
every 1 year when a prior mammogram was unsatisfactory or abnormal, or if
they feel screening should take place more often than 2 years based on 
the patient's history, risk or strong patient preferences.
 
A code for breast cancer or prior abnormal mammogram (VA-WH HX BREAST 
CANCER/ABNORMAL MAM) changes the frequency to q1 year for 5 years as a
safety features so that patients with these diagnoses are not lost to
follow-up.  
       Note: The term VA-WH HX BREAST CANCER/ABNORMAL MAM comes
      individual decision and take the patient's values into account
             mapped with the previously distributed VA-BREAST TUMOR and
             VA-MASTECTOMY taxonomies mapped to this term. This term uses
             computed finding VA-WH MAMMOGRAM ABNORMAL IN WH PKG to search
             for the existence of any abnormal results in the WH Package.
             This 1 year frequency is subsequently overridden if any other
             frequency is chosen by the clinician.
 
In the 2013 update, the taxonomy VA-BREAST TUMOR was edited to exclude 
codes that do not represent 
breast cancer.  The following codes were deleted:
      including values about specific benefits and harms
This taxonomy was updated in 2013 to include only those codes that 
represent cervical cancer or CIN.

Technical Description:
     
         * If Function Findings determine that an information finding 
           exists that alters the baseline frequency to 4M, 6M, 1Y or 2Y,
           the baseline frequency will be overridden.
     
       VA-WH MAMMOGRAM SCREEN IN WH PKG 
           No mapping necessary. This term represents mammogram results 
           documented in the Women's Health (WH) Package. The term is 
           mapped to the VA-WH MAMMOGRAM IN WH PKG computed finding which 
           only uses WH findings that are normal or abnormal.  
     
    This reminder is recommended for use by clinicians at Primary Care 
       VA-WH MAMMOGRAM SCREEN IN RAD PKG 
           Mapping will be necessary.  This term represent Mammogram 
           results documented in the Radiology package.  Map local
           radiology procedures that represent the following procedures:
              MAMMOGRAM BILAT 
              MAMMOGRAM UNILAT 
              MAMMOGRAM SCREEN 
           Each finding should have a condition added to exclude 
           unsatisfactory results.  
     
    Clinics (PACT/Primary Care, Medicine, GIMC, Geriatric, Women's) and
       VA-WH MAMMOGRAM SCREEN DONE 
           Some mapping may be appropriate. This reminder term will use 
           the previously distributed VA-MAMMOGRAM/SCREEN taxonomy to 
           find ICD DIAGNOSIS or CPT coded results.
     
           Use the new WH MAMMOGRAM OUTSIDE health factor to document 
           Mammogram results completed outside the VA when the Mammogram 
           results are not documented in Radiology, Women's Health or 
           Consult packages.

           This reminder term represents the action taken from the dialog 
    patients.

           mammogram screening. The health factor date will be used to 
           calculate the resolution frequency, instead of the Start date 
           related to the order actually placed.

           any local findings that indicate a Mammogram screen is not
           indicated for this patient.
     
           This term is distributed with mapping to the following

           Use the WH MAMMOGRAM DEFERRED or WH MAMMOGRAM DECLINED health 
        * Results, with interpretation, can be entered and verified in 
           factors distributed with this term or add any local health 
           factor representing that mammogram screening should be 
           satisfied for one week.
          
       VA-WH MAMMOGRAM UNSATISFACTORY IN RAD/WH PKG 
           Mapping will be necessary for Radiology. This term represent
           Mammogram results documented in the Radiology and WH packages.
           Map local radiology procedures that represent the following
           procedures:
              MAMMOGRAM BILAT 
    This reminder is based on VHA Clinical Guidance, Performance measures
          the Radiology package.
              MAMMOGRAM UNILAT 
              MAMMOGRAM SCREEN 
           Each finding should have a condition that checks for
           unsatisfactory results.
           
           No mapping is necessary for WH.  Use the VA-WH MAMMOGRAM IN
           WH PKG computed finding and the value "UNSATISFACTORY" to
           find unsatisfactory results documented in the WH package.

    FUNCTION FINDINGS:

    Function Findings (FF) will be used to determine the frequency of 
    this reminder. FF logic has been set up to differentiate between
    completed results and findings that represent different frequencies
    (4M, 6M, 1Y, 2Y).  
    If more than one of the frequency findings are recorded at the same
    date/time, the finding that makes the reminder due most often will

          Branching logic is used in the reminder dialog so that the

          reflect site information and the correct letter content for each
          type of letter. 
        * The WV NOTIFICATION PURPOSE treatment need and treatment need
          date offset should be defined if appropriate. These new fields
          will be used to update Women's Health data when purpose of
          notifications are selected by clinicians from reminder dialogs.
        * The reminder dialog definitions should reference the appropriate
          WV NOTIFICATION PURPOSE.

          the new reminder: VA-WH DISCUSS BREAST CA SCREEN WOMAN 40-49.No
          other health factors need to be mapped to this term.
 
       VA-WH BILATERAL MASTECTOMY 
            VA-*BREAST CANCER SCREEN 
           This term will use the VA-WH BILATERAL MASTECTOMY taxonomy to 
           find coded bilateral mastectomies. The health factor WH 
           BILATERAL MASTECTOMY distributed with this term may be used or 
           add any local health factor that represents the patient had a 
           bilateral mastectomy and no longer needs mammogram screening.  
 
       VA-TERMINAL CANCER PATIENT 
           No mapping necessary.  Use the VA-TERMINAL CANCER PATIENTS
           reminder taxonomy distributed with this term.
      
            VA-MAMMOGRAM 
    RESOLUTION FINDINGS:
