DESCRIPTION | This reminder is due every 6 months for patients who have their last The reminder is resolved for 6 months by any of the interventions listed below: Education (reminder term HTN EDUCATIONAL INTERVENTIONS) VA-HTN EXERCISE EDUCATION (ED) VA-HTN NUTRITION EDUCATION (ED) HTN LIFESTYLE MODIFICATIONS RECOMMENDED (HF) VA-HTN MEDICATION ADHERENCE (ED) HTN NO EDUC INTERVENTION WARRANTED (HF) recorded DBP >89 or the last SBP >139 and the patient has a diagnosis of Medication (reminder term HTN MED & OTHER INTERVENTIONS) HTN MEDICATIONS ADJUSTED (HF) HTN REFUSAL OF MED INTERVENTION (HF) HTN EVALUATION OF RESISTANT/SECONDARY (HF) HTN NO MED CHANGE - BP CONTROLLED (HF) HTN NO MED CHANGE - COMORBID ILLNESS (HF) HTN NO MED CHANGE - SIDE EFFECTS FROM RX (HF) HTN NO MED CHANGE - LIMITED LIFE EXP (HF) HTN NO MED CHANGE - NONCOMPLIANCE (HF) HTN in PCE, problem list or PTF in the past 18 months. HTN NO MED CHANGE - OTHER (HF) The reminder becomes not applicable if the health factor of INCORRECT HTN DIAGNOSIS is entered. The reminder is reactivated the next time that an ICD code for HTN is entered. The following findings are included in the reminder to provide additional information in the clinical maintenance display: Any diabetes diagnoses, drug classes for HTN meds (CV100, CV150, CV400, CV490, CV700, CV709, CV800, CV805), patient's BMI. The reminder becomes not applicable if the last BP <140/90 is recorded. It is also not applicable if the last SBP is >159 or the last DBP is >99 since the "HTN ASSESSMENT BP >160/100" reminder will be applicable for the patient with the higher measured BPs. |
