DESCRIPTION
This reminder is due every 6 months for patients who have their last 
The reminder is resolved for 6 months by any of the interventions listed 
below:

The reminder becomes not applicable if the health factor of INCORRECT HTN 
DIAGNOSIS is entered.  The reminder is reactivated the next time that an 
ICD code for HTN is entered.  
 
The following findings are included in the reminder to provide additional 
information in the clinical maintenance display:
 
Any diabetes diagnoses, drug classes for HTN meds (CV100, CV150, 
 
CV400, CV490, CV700, CV709, CV800, CV805), patient's BMI.
The reminder becomes not applicable if the last BP <140/90 is recorded.  
It is also not applicable if the last SBP is >159 or the last DBP is >99 
since the "HTN ASSESSMENT BP >160/100" reminder will be applicable for 
the patient with the higher measured BPs.
