This reminder is due every 1 month for patients whose last DBP was >99 
     HTN MEDICATIONS ADJUSTED                   (HF)
     HTN REFUSAL OF MED INTERVENTION            (HF)
     HTN EVALUATION OF RESISTANT/SECONDARY      (HF)
     HTN NO MED CHANGE - BP CONTROLLED          (HF)
     HTN NO MED CHANGE - COMORBID ILLNESS       (HF)
     HTN NO MED CHANGE - SIDE EFFECTS FROM RX   (HF)
     HTN NO MED CHANGE - LIMITED LIFE EXP       (HF)
     HTN NO MED CHANGE - NONCOMPLIANCE          (HF)
     HTN NO MED CHANGE - OTHER                  (HF)
 
or the last SBP was > 159 and the patient has a diagnosis of HTN.  
Educational interventions are optional:

The reminder becomes not applicable if the health factor of INCORRECT HTN 
DIAGNOSIS is entered.  The reminder is reactivated the next time that an 
 
ICD code for HTN is entered.  
 
The following findings are included in the reminder to provide additional 
information in the clinical maintenance display:
 
Any diabetes diagnoses, drug classes for HTN meds (CV100, CV150, 
CV400, CV490, CV700, CV709, CV800, CV805), patient's BMI.
The reminder becomes not applicable if a BP lower than 160/100 is entered.
 
The reminder is resolved for 1 month by recording at least one
intervention from the list below.
