DESCRIPTION | This reminder was activated through an algorithm which identifies Health factors are collected for each of the yes/no responses to the 4 questions. A 'yes' response to all 4 questions is a positive. HEALTH FACTORS: POLYTRAUMA OEF/OIF - YES POLYTRAUMA OEF/OIF - NO POLYTRAUMA SINGLE EVENT - YES POLYTRAUMA SINGLE EVENT - NO POLYTRAUMA EVACUATION - YES patients who may have experienced severe Polytrauma due to OEF/OIF combat POLYTRAUMA EVACUATION - NO POLYTRAUMA IMPAIRMENT HISTORY - YES POLYTRAUMA IMPAIRMENT HISTORY - NO Combinations of the following reminder terms and associated taxonomies define which patients may have experienced polytrauma and therefore need to be asked about those injuries. VA-POLYTRAUMA AMPUTATION VA-POLYTRAUMA AUDITORY related injuries. By definition, Polytrauma occurs due to a single event VA-POLYTRAUMA BRAIN INJURY VA-POLYTRAUMA BURN VA-POLYTRAUMA INPT REHAB VA-POLYTRAUMA ORTHO VA-POLYTRAUMA PTSD VA-POLYTRAUMA SCI VA-POLYTRAUMA VISION VA-POLYTRAUMA WAR INJURY The reminder is resolved by a negative response to questions 1-4 or a with injuries to more than one physical region or organ system, one of positive response to question 4. The health factors that represent these responses are included in a reminder term: VA-POLYTRAUMA MARKER COMPLETED which may be life threatening, and which results in physical, cognitive, psychological, or psychosocial impairments and functional disability. The following questions will serve as the clinical confirmation that the injuries experienced by this patient meet the definition of severe Polytrauma related to OEF/OIF combat. |
