This patch will correct the following
 
1. When a patient has changed clinics and is displaying on the team 
   list for the new clinic. They are not being removed from the old
   clinic's team list.
 
2. When listing all patients on a clinic team list using the 
   Anticoagulation Patinet Lists [ORAM PATIENT LIST MENU] option, patients
   whos name begins with the letter Z do not show up.
