This patch will correct the following issues:
4) When a mail message from AudiCare Auto-renwal is alerting about an
invalid Provider status, the provider in question is not known.
1) Parameter definition description missing for OR AUTORENEWAL USER.
2) Renewed prescriptions processed by AUDIOCARE are showing the user
who processed audiocare renewals as the person who entered the
renewal.
3) Mail messages from AudioCare Auto-renewal are not displaying
enough information to identify a patient in CPRS.
